Im currently connecting MT-32 thru game port on AWE64G, do i need to additional game port card to connect my Gravis PC gamepad ?

No. Cables exist that allow a pass through of the joystick and a midi device to be used simultaneously.

Search on eBay for "joystick midi" and you'll see them.

Somehow using both joystick and midi, Only 2 out of 4 button of Gamepad working. I use similar cable to what JCarlos XJ suggestion
